[摘 要] 目的 探討细胞和体液免疫检测诊断脓毒症患者的临床效果.方法 方便选取该院2015年7月—2016年7月收治的脓毒症患者68例,其中脓毒症37例,脓毒症休克31例；另选取同期于该院体检的健康人40名,检测3组CD4、CD8、CD4/ CD8细胞和IgG、IgA及IgM体液免疫水平.结果 脓毒症组CD4CD25Treg水平为（1.70±0.43）%,休克组为（3.55±0.82）%,明显高于健康组（P<0.05）；脓毒症休克组CD3水平为（35.48±3.69）%,明显低于脓毒症组（71.83±8.03）%与健康组（64.21±6.93）%（P<0.05）,脓毒症组与休克组对比差异无统计学意义（P>0.05）；脓毒症组和休克组CD4水平分别为（45.13±2.5）%、（23.23±1.6）%,均低于健康组（P<0.05）,休克组CD8为（11.91±1.3）%,低于健康组（29.23±2.6）%与脓毒症组（29.14±2.7）%（P<0.05）,脓毒症组CD4/CD8为（1.55±0.06）,低于健康组（1.93±0.12）与休克组（1.95±0.04）（P<0.05）；脓毒症IgA为（2.37±0.52）mg/dL,IgG为（1 073.8±27.5）mg/dL,IgM水平为（2.03±0.25）mg/dL；休克组IgA为（2.31±0.41）mg/dL,IgG为（1 093.4±29.4）mg/dL,IgM水平为（1.98±0.21）mg/dL,均低于健康组（P<0.05）.结论 检测脓毒症患者细胞与体液免疫水平可评估其预后.

Clinical Effect of Humor and Assays of Cellular Immunity in Diagnosis of Patients with Sepsis

[Abstract] Objective To study the clinical effect of humor and assays of cellular immunity in diagnosis of patients with sepsis. Methods 68 cases of patients with sepsis admitted and treated in our hospital from July 2015 to July 2016 were conveniently selected including 37 cases with sepsis and 31 cases with sepsis shock, and 40 cases of healthy persons in our hospital for physical examination at the same period were selected, and the CD4, CD8, CD4/ CD8 cells and IgG, IgA and IgM humor immunity levels of the three groups were tested. Results The CD4CD25Treg level in the sepsis group and shock group was respectively（1.70±0.43）% and （3.55±0.82）%, which was obviously higher than that in the healthy group（P<0.05）,and the CD3 level in the sepsis shock group was obviously lower than that in the sepsis group and healthy group, [（35.48±3.69）% vs （71.83±8.03）%, （64.21±6.93）%]（P<0.05）, and the difference between the sepsis group and the shock group was not obvious, P>0.05, and the CD4 level in the sepsis group and shock group was respectively（45.13±2.5）% and （23.23±1.6）%, which was lower than that in the healthy group（P<0.05）, and the CD8 in the shock group was lower than that in the healthy group and sepsis group, [（11.91±1.3）% vs （29.23±2.6）%, （29.14±2.7）%]（P<0.05）, and the CD4/CD8 in the sepsis group was lower than that in the healthy group and shock group, [（1.55±0.06）vs（1.93±0.12）,（1.95±0.04）]（P<0.05）, and the IgA, IgG and IgM levels in the sepsis group and shock group were respectively （2.37±0.52）, （1 073.8±27.5）, （2.03±0.25）mg/dL and （2.31±0.41）,（1 093.4±29.4）,（1.98±0.21）mg/dL, which were lower than those in the healthy group（P<0.05）. Conclusion Testing the immunity levels of cell and humor of patients with sepsis can evaluate the prognosis.
