【摘 要】 目的 探讨根管治疗联合牙周基础治疗对牙周牙髓联合病变的临床效果.方法 86例牙周牙髓联合病变患者作为本次实验的研究对象, 根据治疗方法的不同分为比较组与研究组, 每组43例.比较组患者采用常规的根管治疗, 研究组患者则采用根管治疗联合牙周基础治疗.比较两组患者的治疗效果及不良反应发生情况.结果 研究组术后1个月、6个月、1年的总有效率分别为97.7%、90.7%、83.7%, 均高于比较组的86.0%、74.4%、65.1%, 差异均具有统计学意义（P<0.05）.研究组不良反应发生率9.3%明显低于比较组的25.6%, 差异具有统计学意义（P<0.05）.结论 在牙周牙髓联合病变的治疗中, 采用根管治疗联合牙周基础治疗比单纯采用常规的根管治疗效果更好, 安全性更高, 值得临床推广应用.

【Abstract】 Objective To discuss the clinical effect of root canal therapy combined with periodontal basic therapy on combined periodontal-endodontic lesions. Methods A total of 86 patients with combined periodontal-endodontic lesions as study subjects were divided by different treatment methods into control group and research group, with 43 cases in each group. The control group was treated with conventional root canal therapy, and the research group was treated with root canal therapy combined with periodontal basic therapy. Comparison were made on treatment effect and occurrence of adverse reaction between two groups. Results The research group had higher total effective rate of postoperative 1 month, 6 months and 1 year respectively as 97.7%, 90.7% and 83.7% than 86.0%, 74.4% and 65.1% in the control group, and their difference was statistically significant （P<0.05）. The research group had obviously lower incidence of adverse reactions as 9.3% than 25.6% in the control group, and the difference was statistically significant （P<0.05）. Conclusion During the treatment of combined periodontal-endodontic lesions, combined therapy of root canal therapy and periodontal basic therapy shows better treatment effect than simple conventional root canal therapy with higher safety. It is worthy of clinical promotion and application.

牙周牙髓联合病变是指同一颗患牙既存在牙周病变（牙周炎）, 又存在牙髓病變（牙髓炎）, 可能是牙髓感染引起的, 也可能是牙周感染引起的, 且这两种感染可以互相扩散和影响, 从而导致联合病变的发生[1].该病常让患者感到疼痛难忍, 严重影响患者的生活质量.本文就根管治疗联合牙周基础治疗对牙周牙髓联合病变的临床效果进行探究, 现报告如下.
1 资料与方法
1. 1 一般资料 选取2015年6月～2016年6月来本院治疗的牙周牙髓联合病变患者86例作为本次实验的研究对象, 并根据治疗方法的不同分为比较组与研究组, 每组43例.比较组患者中, 男25例, 女18例, 年龄最大68岁, 最小26岁, 平均年龄（46.5±7.2）岁, 其中, 原发性19例, 继发性24例；研究组患者中, 男21例, 女22例, 年龄最大71岁, 最小24岁, 平均年龄（45.8±8.4）岁, 其中, 原发性20例, 继发性23例.两组患者性别、年龄、发病类型等一般资料比较, 差异均无统计学意义（P>0.05）, 具有可比性.
