【摘 要】 目的 探讨紫杉醇联合顺铂新辅助化疗治疗局部晚期宫颈癌的效果.方法 60例局部晚期宫颈癌患者, 随机分为观察组和对照组, 各30例.观察组术前应用紫杉醇联合顺铂新辅助化疗, 对照组术前不实施化疗而是择期手术, 两组均采用全子宫切除术联合盆腔淋巴结清扫术治疗.比较两组患者的治疗效果、手术时间、术中出血量、淋巴结转移阳性率、 切缘阳性率、宫旁累及阳性率.结果 观察组总有效率为76.7%, 高于对照组的50.0%, 差异具有统计学意义 （P<0.05）.两组患者手术时间和术后出血量比较, 差异均无统计学意义（P>0.05）；观察组术后淋巴结转移、 切缘、宫旁累及阳性率均低于对照组, 差异均具有统计学意义（P<0.05）.结论 紫杉醇联合顺铂新辅助化疗治疗能够提高局部晚期宫颈癌的手术效果, 减少术后淋巴结转移、*切缘、宫旁累及病理阳性情况, 值得借鉴.

Observation on effect of neoadjuvant chemotherapy with paclitaxel and cisplatin on locally advanced cervical cancer LIN Shu. Gynecological Ward, Dalian City Women and Children Medical Center, Dalian 116033, China
【Abstract】 Objective To investigate the effect of neoadjuvant chemotherapy with paclitaxel and cisplatin on locally advanced cervical cancer. Methods A total of 60 locally advanced cervical cancer patients were randomly divided into observation group and control group, with 30 cases in each group. The observation group received neoadjuvant chemotherapy with paclitaxel and cisplatin, and the control group received no chemotherapy but elective surgery. Both groups were treated with total hysterectomy combined with pelvic lymph node dissection. Comparison were made on treatment effect, operation time, intraoperative bleeding volume, positive rate of lymph node metastasis, positive rate of vaginal margin and positive rate of para uterine involvement between two groups. Results The observation group had higher total effective rate as 76.7% than 50.0% in the control group, and the difference was statistically significant （P<0.05）. Both groups had no statistically significant difference in operation time and intraoperative bleeding volume （P>0.05）. The observation group had lower positive rate of lymph node metastasis, vaginal margin and para uterine involvement than the control group, and their difference was statistically significant （P<0.05）. Conclusion Neoadjuvant chemotherapy with paclitaxel and cisplatin can improve surgical effect of locally advanced cervical cancer, reduce positive rate of lymph node metastasis, vaginal margin and para uterine involvement. It is worthy of reference.

宮颈癌是常见妇科恶性肿瘤之一, 主要表现为接触性出血等症状, 手术治疗是其主要手段.研究表明, 单纯的手术治疗宫颈癌的效果有限.在手术前实施化疗干预（新辅助化疗）有助于提高手术切除效果, 提高宫颈癌预后[1, 2].本文选择本院收治的局部晚期宫颈癌患者, 观察紫杉醇和顺铂联合实施新辅助化疗对局部晚期宫颈癌患者的影响.现报告如下.
