[摘 要] 目的 分析营养治疗对炎症性肠病的治疗效果.方法 方便选取该院2014年2月—2016年5月收治的68例成人炎症性肠病患者随机分为实验组(营养治疗)和参照组(常规治疗)各34例,实验组在参照组治疗基础上给予肠内营养治疗、肠外营养治疗或肠内联合肠外营养治疗.记录两组炎症性肠病患者治疗效果、不良反应发生情况、住院时间、治疗费用和治疗前后体质指数,给予统计学分析后比较两组临床效果. 结果 实验组内科治疗总有效率、不良反应发生率、住院时间、治疗费用、治疗后体质指数分别为91.1%、14.7%、(21.5±2.1)d、(5 221.30±27.62)元、(19.6±1.49)kg/m2,均优于参照组85.3%、25.5%、(32.3±1.4)d、(7 135.3±647.92)元、(17.3±1.37)kg/m2,差异有统计学意义(P<0.05).结论 炎症性肠病患者给予营养治疗,可以提高内科治疗总有效率、减少不良反应发生、缩短住院时间、降低治疗费用和改善营养状况,是炎症性肠病治疗的常规手段.

Nutrition Treatment of Inflammatory Bowel Disease

[Abstract] Objective To analyze the effect of nutrition treatment of inflammatory bowel disease. Methods Convenient selection 68 cases of adult patients with inflammatory bowel diseases admitted and treated in our hospital from February 2014 to May 2016 were selected and randomly divided into two groups with 34 cases in each, the experimental group adopted the nutrition treatment, while the control group adopted the routine treatment, and the experimental group were given the enteral nutrition, parenteral nutrition or combined treatment on the basis of the control group, and the treatment effect, occurrence of adverse reactions, length of stay, treatment cost and body mass index before and after treatment of the two groups were recorded, and the clinical effect was compared after the statistical analysis. Results The total medical effective rate, incidence rate of adverse reactions, length of stay, treatment cost and body mass index after treatment in the experimental group were better than those in the control group, [91.1%,14.7%,(21.5±2.1)d,(5 221.30±27.62)yuan,(19.6±1.49)kg/m2 vs 85.3%,25.5%,(32.3±1.4)d,(7 135.3±647.92)yuan,( 17.3±1.37)kg/m2], and the differences were statistically significant(P<0.05). Conclusion The nutrition treatment of patients with inflammatory bowel disease can improve the total medical effective rate, reduce the occurrence of adverse reactions, shorten the length of stay, reduce the treatment cost and improve the nutrition status, which is a routine means in treatment of inflammatory bowel disease.

作為临床消化内科常见病症之一,炎症性肠病主要有:溃疡性结肠炎、克罗恩病,且近年来在国外的发病率逐渐上升,尤其在日本.当前,国内还没有普通人群流行病学资料,但到医院就诊炎症性肠病人数在慢慢增多,经过长时间发展,已慢慢变成消化系统中比较常见的一种疾病.该病最大的特点是病程非常长,发作次数较频繁,发生营养不良的几率非常高.根据相关资料显示,有25%~80%左右的患者会产生不同程度蛋白质热能营养不良,且大约20%~40%的炎症性肠病患者会出现体重减轻的症状[1].而且,炎症性肠病患者在初次诊断时,常常会伴随营养不良等并发症发生,且随着病情的发展进展,临床采用药物或者是手术治疗后,会加大营养障碍,因此,临床展开救治工作时,营养治疗和药物、手术等治疗方法一样重要,贯穿在整个炎症性肠病的治疗中.方便选取该院2014年2月—2016年5月收治的68例炎症性肠患者进行研究,探讨给予或不给予营养治疗对营养状况和治疗效果的影响,现报道如下.
