【摘 要】目的：观察皮牵引配合栀黄止痛散外敷治疗儿童髋关节滑膜炎的临床疗效.方法：将
106例儿童髋关节滑膜炎患者随机分为治疗组和对照组,每组53例.对照组给予皮牵引治疗,治疗组在对照组治疗的基础上加用栀黄止痛散外敷.疗程为2周.观察2组临床疗效和视觉模拟评分法（VAS）评分.结果：治疗组治愈45例,好转4例,无效2例,总有效率为96.08%；对照组治愈36例,好转11例,无效6例,总有效率为88.68%.2组比较,差异有统计学意义（P < 0.05）.治疗1周和2周后,2组VAS评分较治疗前均有改善（P < 0.05）,且治疗组优于对照组（P < 0.05）.结论：皮牵引加栀黄止痛散外敷治疗儿童髋关节滑膜炎效果明显,方法便捷,值得临床推广应用.

Clinical Observation on the Treatment of Children Hip Joint Synovitis by Skin Traction combined with External Application of Zhihuang Zhitong San（栀黄止痛散）

【ABSTRACT】Objective：To observe the clinical effect of skin traction combined with external application of Zhihuang Zhitong San（栀黄止痛散）in the treatment of children hip joint synovitis.Methods：106 children
with hip joint synovitis were randomly divided into a treatment group and a control group,53 cases in each.The control group was given skin traction therapy,while the treatment group,based on the treatment of the control group,was treated with Zhihuang Zhitong San.After 2 weeks.The clinical efficacy and visual analogue scale （VAS）scores of the two groups were observed.Results：In the treatment group,45 cases were cured,
4 cases were improved,and 2 cases were ineffective,the total efficiency being 96.08%；while in the control group,
36 cases were cured,11 cases were improved,and 6 cases were ineffective,the total efficiency being 88.68%.The difference between the two groups was statistically significant （P < 0.05）.After one-week and two-week treatments,VAS scores of the two groups improved compared with those before the treatment （P < 0.05） and the treatment group was better than the control group （P < 0.05）.Conclusion：The curative effect of kin traction combined with external application of Zhihuang Zhitong San is obvious in the treatment of children hip joint synovitis,simple,convenient,and worthy of clinical application.

髋关节滑膜炎又叫髋关节一过性（暂时性）滑膜炎,是骨科临床常见疾病.儿童发病率较高,3～
7岁儿童是高发人群,男性患儿多于女性,男女比例约为2.9∶1[1].临床症状以髋部或膝部疼痛为主,患儿可表现跛行,甚至拒绝行走.其临床治疗方法较多,但如果治疗不及时或不当,往往造成严重后果,可发展为股骨头软骨炎,重者可致股骨头坏死[2].笔者采用皮牵引加栀黄止痛散外敷治疗小儿髋关节滑膜炎,疗效较好,现总结报告如下.
1 临床资料
1.1 一般资料 选取2014年1月至2015年7月在本院骨关节病区就诊的小儿髋关节滑膜炎患者106例,采用随机数字表法分为治疗组和对照组,每组53例.治疗组男40例,女13例；年龄1.5～
17岁,平均（6.3±1.5）岁；左髋30例,右髋18例,
