[摘 要] 目的 观察分析高频电刀的使用对混合痔外剥内扎术后切口愈合的影响.方法 方便选择2016年4月—2017年3月该科50例存在3个类似大小混合痔的患者作为临床观察对象,其中,1个部位为电刀组,采取高频电刀切割组织并电凝止血；1个部位为电凝组,使用常规手术剪剥离组织并用电刀电凝止血；另1个部位为对照组,用手术剪常规剥离组织后用可吸收线缝扎止血,对3组切口术后愈合情况进行对 析.结果 ①电刀组治愈率为56.0%、电凝组治愈率为52.0%、对照组治愈率为50.0%,组间差异无统计学意义（P>0.05）；②电刀组切口均在术后25 d内完全愈合,且整体愈合时间和电凝组、对照组相比,差异无统计学意义（Z等于0.758,P等于0.686）；③电刀组创面平整度为74.0%,电凝组为78.0%,对照组为84.0%,組间差异无统计学意义（P>0.05）；④电刀组术后切口感染率为12.0%、皮瓣坏死率为8.0%略高于电凝组和对照组,但差异无统计学意义（P>0.05）.结论 在混合痔外剥内扎术中,正确应用高频电刀切割组织对术后切口愈合没有明显的影响,同时还可明显缩短手术时间,减少术中出血量.

Clinical Study on the Effect of High Frequency Electric Knife on Wound Healing of Mixed Hemorrhoids

[Abstracts] Objective This paper tries to observe the effect of high frequency electric knife （HFEK） on incision healing after mixed hemorrhoid surgery. Methods 50 patients with three parts of mixed hemorrhoids in this hospital from April 2016 to March 2017 were convenient selected, one site for HFEK group to take HFEK cutting tissue and coagulation hemostasis； one site for electrocoagulation group using conventional surgical shear tissue and coagulation hemostasis； one site for the control group with surgical scissors routine detachment of the tissue and suture bleeding with absorbable line. The postoperative wound healing conditions of three groups were compared. Results ① The curative rate of electric knife group was 56.0%, the electrocoagulation group was 52.0%, and the control group was 50.0%, the difference between the groups was not statistically significant（P>0.05）； ②The incisions by electric knife were all recovered in 25 days, there was no significant difference of the overall healing time between the electric knife group, the electrocoagulation group and the control group（z等于0.758, P等于0.686）； ③The rate of wound smoothness of electric knife group was 74.0%, electrocoagulation group was 78.0%, and the control group was 84.0%, the difference between the groups was not statistically significant（P>0.05）； ④ The incision infection rate was 12.0% and the flap necrosis rate was 8.0% in the electric knife group, higher than the electrocoagulation group and the control group, but there was no significant difference between the three groups （P>0.05）. Conclusion In the mixed hemorrhoid surgery, the correct application of high-frequency electric knife cutting tissue on postoperative incision healing do not have significantly effect, but can significantly shorten the operation time, reduce the amount of intraoperative blood loss.
