摘 要 目的：为制定舒缓疗护收费标准提供依据.方法：选取2016年上海市某老年护理院舒缓疗护病区晚期恶性肿瘤出院患者42例,和同时期一般老年护理病区出院患者123例,对两组患者的住院费用进行比较分析.结果：舒缓疗护病区患者的人均住院天数明显低于同期一般老年护理病区,而舒缓疗护病区患者的日均费用、日均药费明显高于同期一般老年护理病区,其差异均有统计学意义（P<0.05）.结论：舒缓疗护病区晚期恶性肿瘤临终患者与一般老年护理患者住院费用有差异,应将相应的服务内容列入到医疗保险收费项目中.

Analysis and study of palliative nursing and hospitalization expenses of elderly patients nursed in an elderly nursing home

ABSTRACT Objective： To provide the basis for setting up the charge standard of palliative care. Methods： In 2016, 42 patients with advanced malignant tumors discharged from an elderly nursing home in Shanghai and 123 cases discharged from the general nursing wards at the same time were selected, and the hospitalization expenses of the two groups were compared and analyzed. Results： The per capita hospitalization days of patients in the palliative care words were significantly lower than those in the general elderly nursing wards but the daily cost and daily average charge of patients in the palliative care words were significantly higher than those in the elderly nursing wards and the differences were statistically significant（P<0.05）. Conclusion： The cost of hospitalization of terminal patients with advanced malignant tumors in the palliative nursing wards is different from that of the general elderly patients nursed, and the corresponding service content should be included in the medical insurance fee projects.

在上海市人口日益老龄化、癌症成为人口死亡重要原因的前提下,临终患者对于舒缓疗护的需求越来越迫切.世界卫生组织（WHO）指出,舒缓疗护是一系列照护方法,它通过运用早期确认、准确评估和完善治疗身体病痛和心理及精神疾患来干预并缓解患者痛苦,以此提高罹患威胁生命疾病的患者及其家属的生活质量[1].
上海市黄浦区顺昌医院被列入2014年上海市政府实事工程项目,开展第二批舒缓疗护病区建设.为分析舒缓疗护病区晚期恶性肿瘤患者和一般老年护理病区患者住院费用的差别,以上海市黄浦区顺昌医院2016年舒缓疗护病区晚期恶性肿瘤出院患者為研究组,和同期一般老年护理二病区出院患者的住院费用情况进行比较分析,为政府制定舒缓疗护服务内容和收费标准提供依据.
1 资料和方法
1.1 一般资料
选取2016年上海市某老年护理院舒缓疗护病区晚期恶性肿瘤出院患者42例作为研究组,选取同时期一般老年护理病区出院患者123例作为对照组,对两组患者的住院费用进行比较分析.
研究组患者42例,其中男性9例,女性33例；年龄52～98岁,平均年龄（81±10）岁；患者罹患消化道肿瘤（食管癌、胃癌、肠癌、肝癌、胰腺癌）13例,肺癌8例,乳腺癌4例,颅内肿瘤10例,其他7例；出院时好转10例,未愈9例,死亡23例.
对照组患者123例,其中男性43例,女性80例；年龄58～93岁,平均年龄（83±8）岁；患者罹患脑梗死31例,痴呆77例,冠心病9例,其他6例；出院时好转106例,死亡17例.
1.2 方法
从医院计算机系统中导出2016年舒缓疗护病区出院的晚期恶性肿瘤患者和同期一般老年护理二病区出院患者的基本资料及住院期间所有费用数据,包括患者疾病种类、住院天数、日均费用及日均费用的具体组成情况.
