摘 要 目的:统计并分析上海市闵行区6家社区卫生服务中心2014—2015年门诊抗菌药物使用情况.方法:选取闵行区6家社区卫生服务中心门诊处方648.45万张,对处方中的药名及剂型、类别、销售金额、用药频度(DDDs)及日均费用(DDC)等进行统计分析.结果:门诊处方中,2年门诊处方中含抗菌药物的处方数为60.35万张,占总处方的9.31%;2014年抗菌药物总处方数30.17万张,占总处方的9.54%,2015年抗菌药物总处方数30.18万张,占总处方数的9.09%;2014、2015年各级抗菌药物DDDs差异不大,维持基本平稳状态;2014、2015年头孢菌素类使用金额分别占抗菌药物总金额的57.55%、59.51%,第2、第3位分别为大环内酯类及喹诺酮类;DDDs排序依次为头孢菌素类、喹诺酮类和大环内酯类.结论:通过对社区卫生服务中心门诊抗菌药物应用数据分析,希望能为临床用药提供参考,进一步提高合理使用抗菌药物水平.

Analysis of application data of antimicrobial agents in the clinics of the six community health service centers in Minhang District*

ABSTRACT Objective: To summarize and analyze the use of antibacterial agents in the clinics of the six community health service centers in Minhang District, Shanghai from 2014 to 2015. Methods: A total of 6.4845 million prescriptions were selected from the outpatient clinics of the six community health service centers in Minhang District. The drug names, forms, categories, sale values, defined daily doses(DDDs) and the defined daily cost(DDC) were statistically analyzed. Results: The number of prescription drugs containing antimicrobials was 603 500 in the outpatient prescriptions of 2 years, accounting for 9.31% of the total prescriptions. In 2014, the total number of antimicrobial agent prescriptions was 301 700, accounting for 9.54% of the total number of prescriptions. In 2015, the total number of antimicrobial agent prescriptions was 301 800, accounting for 9.09% of the total number of prescriptions. In 2014 and 2015, there were no significant differences in the levels of antimicrobial DDDs which maintained the basic steady state. In 2014 and 2015, the amount of money of cephalosporins accounted for 57.55% and 59.51% of the aggregate amount of antimicrobial agents, respectively. The second and third antimicrobial agents were macrolides and quinolones. DDDs were ranked in the descendant order from cephalosporins, quinolones to macrolides. Conclusion: Through the analysis of the application data of antibacterial agents in the clinics of the community health service centers, it is hoped to provide a reference for clinical medication for further improving the level of the rational use of antimicrobial drugs.

抗菌藥物是指具有杀灭或抑制各种病原微生物作用,用于防治细菌感染的药物.抗菌药物品种繁多、临床应用广泛,销售额占比也较大,中国滥用抗生素的问题日益严重[1].因此,如何合理使用抗菌药物,如何更有效地发挥抗菌药物的治疗作用,最大限度地防止或减少不良反应及耐药性的产生,提高合理使用抗菌药物的水平,已成为临床用药的关键问题.本研究对上海市闵行区6家社区卫生服务中心2014—2015年抗菌药物使用情况进行统计,希望为临床用药提供参考,促进抗菌药物的合理使用.
